Ternion Physician Group is the physician group responsible for medical oversight at your facility. Our physicians, nurse practitioners, and physician assistants visit daily, manage medications, coordinate specialist care, and communicate with your family throughout the stay. We are not the facility — we are the physician team inside it.

Daily Physician Rounding

A Ternion Physician Group physician, NP, or PA visits your facility every day to assess patients, review medications, and respond to any changes in condition — before they become emergencies.

A Dedicated Medical Director

Every Ternion Physician Group facility has a named Medical Director who leads the clinical team and is personally accountable for the quality of care patients receive.

On-Site Specialty Care

When your loved one needs specialized care — for complex medical conditions, wound management, or behavioral health support — our team coordinates directly with the facility so care happens on-site, not through an outside referral.

Transition Planning

Our physicians work closely with discharge planners and families to ensure a safe, well-coordinated transition — whether returning home, to assisted living, or to another level of care.
