Ternion Physician Group is a physician-led practice dedicated to post-acute and long-term care across Arizona. We believe every patient in a skilled nursing or rehab facility deserves a doctor who is truly present — not just available by phone.
Our Mission
Better care after the hospital.
Too many patients leave the hospital and end up in a post-acute facility where physician oversight is inconsistent, reactive, or hard to access. Ternion Physician Group was founded to change that — by placing physicians on-site every day, managing complex conditions proactively, and staying involved through every transition of care. Our goal is simple: help patients recover safely, go home sooner, and stay home.
Daily Presence
Our physicians round every day — not on-call, not reactive. Consistent presence means problems get caught early and patients always have a doctor who knows their case.
Physician Accountability
Every facility we serve has a dedicated Medical Director who leads the clinical team and is personally accountable for the quality of care patients receive.
Coordinated Care
We work alongside nursing, therapy, social work, and families — not in isolation. Good post-acute care is a team effort, and our physicians are active participants in every part of it.
Safe Transitions Home
Getting patients home safely is the goal. We manage every transition — from hospital to SNF, SNF to home — with the same attention and follow-through we bring to daily care.

A Named Medical Director
Every facility has a dedicated Medical Director — a working physician who leads the clinical team, attends care meetings, and is accountable for the standards of care you receive.
Daily Physician Rounding
Our physicians and advanced practice providers visit every day. Your condition is assessed regularly, your medications are reviewed, and your care plan is updated to reflect where you are in your recovery.
On-Site Specialty Care
Internal medicine, wound care, and behavioral health support are available on-site — so you get the care you need without the stress of outside appointments or transfers.
Transition Planning and Follow-Through
When it is time to go home, we manage the transition — coordinating follow-up care, reconciling medications, and making sure you leave with a clear plan and the support to stay well.

Physician-directed wound assessment, treatment planning, and ongoing management for pressure injuries, diabetic ulcers, and post-surgical wounds — coordinated with nursing for consistent outcomes.
Behavioral Health
Support for patients experiencing depression, anxiety, or behavioral changes during a post-acute or long-term stay — integrated into the overall care plan so the whole patient is cared for.
